About the Role
As a Community Support Specialist, you will manage a caseload of 20-30 clients, providing individualized services based on their strengths and needs. Your responsibilities will include supporting clients in achieving their goals, communicating with family members and other professionals, aiding in accessing public services, and providing education in daily living skills. You will also participate in discharge planning for hospitalized clients and advocate for their needs while maintaining accurate documentation.
About You
Required:
- Meets the criteria for a qualified mental health professional or has relevant experience based on educational background.
- At least one year of experience in a related field if a non-human service Bachelor’s degree is the highest education obtained.
Preferred:
- Experience in case management with adults and/or youth with severe and persistent mental illness.
- Bachelor’s degree in a human service field.
