Summary

Community Solutions, Inc. (CSI) is seeking a Community Transition Coordinator to join their Chase Center program located in Waterbury, CT. The role involves managing client discharge planning and facilitating community transitions for clients involved in the criminal justice system.

About the Role

As a Community Transition Coordinator, you will be responsible for all aspects of client discharge planning, including coordinating exit requirements with Probation and Parole Officers, providing community-based referrals, and collecting and reporting data on services. You will also participate in performance management meetings and conduct post-release follow-ups to ensure clients successfully reintegrate into the community.

About You

Required:

Associates Degree in a social science field and one year experience in social services or similar field.
Excellent oral, written and interpersonal skills.
Experience with Microsoft Office.
Valid driver's license and transportation.
