About the Role
As a Medical Coordinator/LVN, you will facilitate the hospital discharge process, perform medical assessments, maintain guest charts, conduct wellness checks, and coordinate medical appointments. You will also participate in case conferencing and provide advocacy for guests, managing a caseload of 15 to 20 individuals while adhering to NHF policies and procedures.
About You
Required:
- Current LVN License or bachelor’s degree in Nursing.
- Current American Heart Association BLS (CPR/First Aid/AED).
- Up-to-date TB test and immunizations.
- Proven ability to stay calm and focused under stressful situations.
- Effective communication and social skills.
Preferred:
- Recent work experience with homeless patients.
- Bilingual in Spanish.
- Experience working with substance abuse and dually diagnosed guests.
