About the Role
As a Discharge Planner, you will be responsible for coordinating discharge planning services, collaborating with agencies such as the Conservator’s Office, Public Defender, and County Mental Health Services. You will work full-time, Monday to Friday, from 8:00 AM to 5:00 PM, ensuring that clients receive the necessary support for their recovery.
About You
Required:
- Two (2) years of professional experience in Social Work, Nursing or a related field.
- Must be at least 18 years of age.
- Must be CPR, Crisis Prevention Institute (CPI), and First Aid certified on date of employment or within 60 days of employment.
- Valid and current driver’s license, and personal vehicle insurance with your name listed as a driver.
- Willingness to use your personal vehicle to attend meetings.
