About You
- Bachelor’s Degree from an accredited college or university in a related field; applicable experience may be substituted for a degree.
- 3-5 years clinical case management/discharge planning experience.
- Experience in creating and maintaining relationships with referral sources and community agencies.
- Detail-oriented with the ability to carry out tasks with all necessary details.
