Summary
Strive Health Services LLC is seeking an Engagement Coordinator to join their team in Detroit, MI. The role involves initiating and maintaining patient engagement with Strive's comprehensive care program.
About the Role
As an Engagement Coordinator, you will be responsible for proactively reaching out to patients via phone, educating them on available care services, and connecting them with appropriate resources. You will also research and update patient information, build internal relationships to support patient needs, and develop long-term relationships with patients through effective communication. This role may require in-person visits to patients' homes or local physician offices.
About You
Required:
- 2+ years combined of related education, experience, or certification.
- CPR Certification required.
- Efficient and reliable transportation, including an active driver’s license.
Preferred:
- Customer service experience.
- Intermediate proficiency in MS Word, Excel, PowerPoint, and Outlook.
- Experience navigating electronic medical records.
- Experience communicating with stakeholders across the healthcare ecosystem.
- Experience with enrolling or selling health care services to patients.
