About the Role
As a Family Partner, you will work with program participants in the Family Partnership Program and the Mobile Postpartum Care Unit, providing skills-based coaching and acting as a bridge between participants and healthcare services. Your responsibilities include enrolling individuals, conducting assessments, delivering health education, and supporting navigation of care and resources. The position requires flexibility in work location and hours, including some evening and weekend availability.
About You
Required:
- High School Diploma required.
- Experience working as a case manager, patient navigator, or community health worker strongly preferred.
Preferred:
- Experience working as a medical assistant and/or in a clinical setting.
- Bilingual in Spanish and English.
