Summary

About the Role
As a Family Service Coordinator, you will coordinate intakes, evaluations, and assessments to determine eligibility, facilitate the development of Individual Family Service Plans (IFSPs), support families in accessing community resources, and maintain accurate child files in compliance with standards. You will also facilitate transition planning for children exiting the program and manage a community resource register.
About You
Required:
- Bachelor's degree in Social Work, Psychology, Nursing, Special Education, Early Childhood Education, Counseling, or related field.
- Valid driver's license required.
- Background check and driving record check required.
Preferred:
- Bilingual skills are a plus!
