TAG MedStaffing is seeking a Field Outreach Coordinator (Home Care) to join their team in Bronx, NY. The role involves in-person outreach, patient support, and relationship-building to promote home care services.
About the Role
As a Field Outreach Coordinator, you will engage with the community by attending events and visiting senior centers to promote services. You will assist patients with evaluations, deliver documents, and collect intake forms in their homes. Building strong relationships with community partners and referral sources is essential, as well as sharing real-time updates with the intake and marketing teams. You will also help collect insurance information and physician confirmations to support the intake process.
About You
Required:
- Bilingual in Spanish and English is required.
- High school diploma or equivalent is required.
- 1+ year of experience in healthcare, home care, community outreach, or a similar field-facing role.
- Comfortable using smartphones, email, and basic applications for communication and documentation.
- Excellent interpersonal, organizational, and customer service skills.
- Ability to work independently and manage time effectively.
- Must be comfortable traveling throughout the Bronx for in-home visits and field activities.
Benefits
- Competitive salary of $40,000/year.
- Full-time schedule: Monday to Friday, 9:00 AM – 5:00 PM.
