About the Role
As a Home Care Physical Therapist at Holy Cross Home, you will assess patients' functional status and physical therapy needs in their homes, develop and implement individualized care plans, provide direct physical therapy services according to physician's plans, and educate patients, families, and caregivers on exercises and therapy techniques to promote recovery, mobility, and quality of life.
About You
Required:
- Current licensure as a Physical Therapist in the State of Maryland
- Graduate of an accredited physical therapy program
- At least one year of clinical experience (home care experience preferred)
- Valid driver's license and reliable transportation
- Ability to work independently and manage time effectively
