The Institute for Human Services, Inc. is seeking a HICM+ Case Manager to join their team in Honolulu, HI. The role involves managing a caseload of consumers and providing intense case management services for the homeless SMI population.
About the Role
As a HICM+ Case Manager, you will work collaboratively with the treatment team to implement a client-centered master individualized recovery plan (MIRP) and provide quality case management services including supportive counseling, outreach, and crisis support. You will engage with consumers regularly, coordinate care with other providers, and participate in crisis intervention services.
About You
Required:
- Bachelor's degree in social work, psychology, human services, or a related field with minimum 12 semester credit hours in relevant courses.
- Minimum of 1.5 years of clinical experience in assessing, planning, and providing treatment and support services to clients with severe and persistent mental illnesses.
- Access to a vehicle and valid driver's license with clean abstract.
Preferred:
- Working knowledge of community resources for homeless individuals and substance abuse treatments.
- Effective problem-solving and organizational skills.
