About the Role
As a Home Care Physical Therapist, you will accept and treat patients within your designated geographic area, evaluate their needs for physical therapy in the home setting, and develop customized therapy programs. You will also instruct family members and caregivers on therapy techniques, provide documentation for each visit, and communicate effectively with patients and interdisciplinary team members.
About You
Required:
- Physical Therapist Program Graduate
- Licensed Physical Therapist in the state of New York or eligible
- Mandatory Certifications: First Aid, AED, & CPR Certifications
Preferred:
- Home health experience
- Ability to travel to patients’ homes within the assigned geographic territory
