About the Role
As a Home Health Occupational Therapist, you will provide comprehensive therapy services, including assessment, intervention, care planning, and discharge planning. You will evaluate clients' functions, develop individualized care plans, and educate clients and families on treatment goals. Additionally, you will manage a caseload, coordinate home visits, supervise Home Health Aides, and ensure documentation is completed in line with agency guidelines.
About You
Required:
- Bachelor's degree in Occupational Therapy or Master's degree in Occupational Therapy.
- Current, unencumbered state license as an Occupational Therapist.
- National Provider BLS certification within 30 days of hire.
Preferred:
- 1 year of Occupational Therapy experience with adults in a home care setting.
