About the Role
As a Home Health PDN, you will be responsible for delivering professional nursing care under the direction of a Registered Nurse. Your duties will include providing skilled nursing visits, following care plans, and communicating changes in client conditions. You will work in a supportive environment with a focus on client-centered care, ensuring all daily living needs of the patient are met.
About You
Required:
- Graduate of an accredited school of professional nursing.
- Current license to practice as a Licensed Practical Nurse/Registered Nurse in the state of Ohio.
- Minimum of one (1) year of experience as a PDN in a home healthcare or acute care setting.
- Experience in trach care, G-tube care, Trilogy/vent care, medication administration, and tube feedings.
Preferred:
- Active CPR and First Aid certificates.
- Good interpersonal communication skills - written and verbal.
- Current driver's license and reliable transportation.
- Desirable character traits: reliable, honest, professional, compassionate, detail-oriented, accountable, friendly, and respectful.
