About the Role
As a Home Health Physical Therapist, you will consult, evaluate, plan, and administer Physical Therapy services prescribed by a physician. You will collaborate with an interdisciplinary team to ensure quality care and manage primary patient care delivery for a select group of patients. Your responsibilities will include maintaining open communication with all members of the care group and supporting the patient-centered care model.
About You
Required:
- Current PA licensure in Physical Therapy.
- CPR certification required.
Preferred:
- Bachelor or Master Degree in Physical Therapy.
- One year clinical experience and recent home care experience.
