Millennium Physician Group is seeking a Home Health RN Fulltime to provide nursing care in accordance with the client's Plan of Care in Venice, FL.
About the Role
As a Home Health RN, you will coordinate total patient care by conducting comprehensive health and psychosocial evaluations, monitoring clients' conditions, promoting preventive practices, and coordinating services and educational activities. You will evaluate the effectiveness of nursing services, consult with physicians regarding care plans, participate in case conferences, and ensure continuity of care with other health team members. Additionally, you will maintain clinical records, participate in discharge planning, and strive to improve nursing skills through ongoing education.
About You
Required:
- Current Registered Nurse (RN) license in the state of Florida.
- Experience in home health care or a similar nursing environment.
Preferred:
- Bachelor's degree in Nursing (BSN).
- Certification in Home Health Nursing.
