Hospice of the Western Reserve is seeking a HOPE Licensed Practical Nurse (LPN) to join their team in Fairview Park, OH. The role involves providing prescribed medical treatment and personal care services to terminally ill persons in collaboration with a transdisciplinary team.
About the Role
As a HOPE Licensed Practical Nurse, you will be responsible for assessing the physical, psychosocial, and spiritual needs of patients and their families, providing skilled nursing services, monitoring pain and symptoms, and offering emotional support. You will also coordinate care plans, participate in care conferences, and ensure compliance with clinical standards and policies.
About You
Required:
- Licensed in the State of Ohio.
- Current CPR certification.
- Experience in hospice care, home health, oncology, and/or pain and symptom management.
Preferred:
- Hospice and Palliative Care Certification desired, to be obtained within the first two years of hire.
- Valid driver's license with a good driving record.
