About the Role
As a Hospice Medical Social Worker (MSW), you will assess the psychosocial status of patients and families, provide counseling, and assist in the development of individualized care plans. You will also educate patients and families on advanced directives, communicate with healthcare providers, and coordinate care with community agencies. Your responsibilities will include crisis intervention, maintaining clinical documentation, and participating in on-call duties as needed.
About You
Required:
- Master's degree in Social Work (MSW) from an accredited program.
- Minimum of one year experience in healthcare, preferably in hospice care.
- Good verbal and written communication skills.
Preferred:
- Experience in hospice care.
- Understanding of hospice philosophy and issues related to death and dying.
