About the Role
As a Hospice Nurse Practitioner, you will conduct visits with hospice patients, complete physical exams, assess eligibility for hospice, and communicate with patients, families, and caregivers. You will work independently while attending IDG meetings and prescribing necessary medications and interventions.
About You
Required:
- Valid ROLE certification in the state of job location.
- 1-2 years of primary care experience with older adults.
- Strong people and rapport building skills with a positive demeanor.
- Excellent communication and collaboration skills.
Preferred:
- Demonstrated strong and confident presentation skills.
