About the Role
As a Hospice Patient Care Coordinator, you will coordinate and monitor the day-to-day delivery of patient care services, collaborate with interdisciplinary team members to develop individualized care plans, and provide clinical supervision to nursing staff. You will also offer emotional support to patients and families, assist in quality assurance, and provide ongoing education and training to hospice staff.
About You
Required:
- Current Registered Nurse (RN) license in the state of Texas.
- Minimum of 2 years nursing experience, preferably in hospice or palliative care.
Preferred:
- Bachelor of Science in Nursing (BSN).
- Certification in Hospice and Palliative Nursing (CHPN).
- Experience in care coordination case management or supervisory roles.
