Hospice RN Case Manager

Absolute Health Care is seeking a Hospice RN Case Manager to join their team in Canton, OH. The role involves providing direct patient care services to hospice patients and managing an interdisciplinary group to enhance the quality of hospice care.

As a Hospice RN Case Manager, you will be responsible for ensuring the highest quality of care is provided and documented according to the Plan of Care. Your duties will include performing initial and ongoing assessments of patients, developing and implementing hospice plans of care in collaboration with physicians and team members, and educating patients and their families on medication administration, dressing changes, and hospice philosophy.
