About the Role
As a Hospice Social Worker - PRN, you will be responsible for assessing and addressing the psychosocial needs of clients and their families, documenting all interactions, and facilitating bereavement counseling and groups. You will collaborate with other team members to ensure integrated care and utilize community resources effectively.
About You
Required:
- Masters in Social Work
- 2 or more years in a Social Worker related field, preferably in Community-Based Services and/or Healthcare Setting
- Licensed Masters Social Worker (LMSW) - State Board of Behavioral Sciences upon hire
- CPR/AED/BLS certification - Other BLS within 14 days
Preferred:
- 3 or more years of experience in a related field
