About the Role
As a Hospital Liaison, you will be responsible for following up daily on residents from multiple Aliya Healthcare facilities who are currently in the hospital. Your daily tasks will include conducting visits to hospitals, monitoring the condition of hospitalized residents, communicating with hospital staff, and coordinating with facility staff for smooth transitions back to the facility. You will also advocate for residents' needs and maintain records of all hospital visits and interactions.
About You
Required:
- A minimum of 1 year of experience as a Hospital Liaison.
- Strong understanding of post-acute care and hospital discharge processes.
Preferred:
- Compassionate and detail-oriented.
- Highly organized with excellent communication skills.
