Summary
Evergreen Health is seeking a Housing Retention Counselor to join their team in Jamestown, NY. The role involves providing independent living and retention counseling services to individuals transitioning from homelessness to permanent housing.
About the Role
As a Housing Retention Counselor, you will assist patients with intakes, assessments, move-in activities, and manage financial assistance funds. You will provide stabilization services in collaboration with community partners, conduct housing searches, and ensure patients maximize their access to supportive services. Your responsibilities will also include building relationships with landlords, conducting home visits, and maintaining timely data entry in the Electronic Health Record.
About You
Required:
- Bachelor's degree in Human Services or related field and one (1) year of qualifying experience OR an Associate degree in Human Services or related field and two (2) years of qualifying experience.
- Professional case management or care coordination experience with populations such as persons with chronic illness, mental illness, homelessness, or chemical dependence.
Preferred:
- Experience developing and implementing direct client services and case management services.
- Experience working with families.
