About the Role

As an Inpatient Case Manager MSW, you will be responsible for assessing the social, psychological, cultural, environmental, and financial situations of patients. You will collaborate with patients, families, healthcare team members, and community organizations to identify needs and resources, and develop effective transition plans based on patient diagnosis and treatment goals. This position requires working full-time on weekends with 12-hour shifts.

About You

Required:

Master's Degree in Social Work (MSW)
1 year of acute care and/or healthcare experience (Education internship experience in an acute care facility can be considered)
Certification in Case Management (CCM, CMAC or ACM-SW) required within one year of eligibility
BLS required within 90 days of hire
