Inpatient Social Work, Care Transitions

Beth Israel Lahey Health is seeking an Inpatient Social Work, Care Transitions to join their team in Burlington, MA. The role involves collaborating with the interdisciplinary care team to coordinate post-acute care services and provide psychosocial support to patients and families.

As an Inpatient Social Worker, you will assess high-risk psychosocial factors affecting patients and families, educate the care team on social drivers of health, develop psychosocial assessments and intervention plans, and participate in multidisciplinary rounds. You will also provide crisis intervention, maintain documentation, and facilitate communication with patients and families regarding care transitions.
