About the Role
As an Integrated Health and Outreach Case Manager, you will be part of a multi-disciplinary patient care team, responsible for supporting treatment goals through case management services. You will represent the company positively, ensuring discretion and professionalism in all interactions. Your tasks will include coordinating care across various agencies, maintaining detailed documentation, and advocating for patients' needs while navigating complex service systems.
About You
Required:
- Bachelor's degree in public health, social work, or related field.
- Two years of related case management experience.
Preferred:
- Master's degree in public health or similar field.
- Experience in HIV and STI prevention or community health programs.
- Bilingual (Spanish/English).
- Experience in case management or care coordination in healthcare or community outreach settings.
- Experience drawing blood or willingness to become a certified Phlebotomist.
- Experience in NextGen EHR.
