About the Role
As an Intensive Care Coordinator, you will provide individualized support and planning in a collaborative, person-centered, and strengths-based manner. Your responsibilities will include meeting with families, leading planning processes, collaborating with Montgomery County Children and Youth staff, documenting efforts, preparing reports, and planning for discharge from care coordination.
About You
Required:
- Master’s degree in social services or related field.
- 1 or more years of experience with team dynamics and systems work, e.g. family systems.
- 3 or more years of comprehensive child-serving systems experience.
Preferred:
- Excellent case management, leadership, partnering and customer service skills.
- Expertise with conflict resolution and management.
- Receive training in High Fidelity Wraparound Care Coordination model.
