Summary
Chinatown Community Development Center is seeking an Intensive Case Manager - Transbay 2 West to provide comprehensive case management services to formerly homeless seniors in San Francisco.
About the Role
As an Intensive Case Manager, you will deliver intensive case management services to residents at TransBay Block 2 West, conduct assessments, coordinate with healthcare providers, and facilitate support groups. You will also assist residents in accessing benefits and maintaining housing stability while working within a trauma-informed care model.
About You
Required:
- Bachelor's Degree in Social Work or related field.
- Three years of experience providing intensive case management services for very low-income populations.
- One year of experience working with seniors.
- Knowledge of trauma-informed care, substance use, and mental health issues.
Preferred:
- Master of Social Work or related field.
- Bilingual and biliterate in English/Spanish or Cantonese.
- Experience in a Permanent Supportive Housing environment.
- Familiarity with local resources and public benefits.
