About the Role
About You
- Master's Degree in Social Work (MSW) from an accredited graduate school.
- Current license as a Social Worker by the PA Board of Social Work Examiners.
- Licensed driver with a reliable automobile and insurance.
- Experience in hospice or home care settings.
- Understanding of hospice philosophy and principles of death/dying.
- Ability to assess patient needs and formulate individualized care plans.
