About the Role
As a Licensed Clinical Professional Counselor (LCPC), you will conduct initial diagnostic assessments, develop individualized treatment plans, and provide psychotherapy services tailored to resident needs. You will deliver individual, group, and family counseling, document sessions in compliance with Medicare and facility requirements, and collaborate with the interdisciplinary team to ensure coordinated care.
About You
Required:
- Master’s degree in Counseling, Clinical Mental Health Counseling, Psychology, or a related behavioral health field from a regionally accredited institution.
- Active Illinois LCPC license (or LCSW).
- Enrolled or eligible for enrollment as a Medicare Part B provider.
Preferred:
- Experience working with geriatric populations or in long-term care.
- Strong interpersonal, documentation, and communication skills.
- Ability to work independently within a collaborative, team-based environment.
