About the Role
As a Licensed Clinical Social Worker, you will be responsible for conducting bio-psychosocial assessments, crisis intervention, and participation in multidisciplinary rounds. You will provide patient and family support and education, referrals to community-based services, and assist in discharge and transition planning. This position requires a thorough understanding of the clinical, financial, administrative, and psychosocial components of care management.
About You
Required:
- California Licensed Clinical Social Worker (LCSW)
- Master's Degree in Social Work
- Certification in CMC, ACM or ANCC (within 12 months of hire or eligibility)
Preferred:
- Bilingual
- 3+ years pediatric healthcare experience
