About You
- Master's degree from a school of social work accredited by the Council on Social Work Education.
- Certification or licensure in the state of practice.
- At least one year of social work experience in a healthcare setting.
- Basic technology skills, including operation of a mobile device.
- Experience in Medicare home health or hospice settings.
