Summary
Elevance Health is seeking a Licensed Placement Support Coordinator to join their team in Fayetteville, NC. The role involves supporting Medicaid-enrolled families in the child welfare system by ensuring appropriate foster home placements.
About the Role
As a Licensed Placement Support Coordinator, you will perform case management through telephonic and home visits, assess and identify suitable foster home placements, develop care plans, facilitate pre-placement visits, and maintain relationships with foster families. You will also coordinate with resources to meet the integrated care needs of members and ensure compliance with organizational and legal requirements.
About You
Required:
- Active and current license as an LCSW, LCMHC, LPA, LMFT, or RN issued by the state of North Carolina.
- MS/MA in social work, counseling, or a related behavioral health field, or a degree in nursing.
- Three (3) years of experience providing care management, case management, or care coordination to individuals served by the child welfare system.
Preferred:
- Previous experience in foster care, social work, or related fields.
- Strong understanding of the foster care system and child welfare policies.
- Excellent interpersonal and communication skills, with an ability to work collaboratively.
- Proficiency in Microsoft Office Suite and experience with child welfare databases.
- Empathy, compassion, and a strong commitment to child welfare.
