About the Role
As a Licensed Practical Nurse, you will be responsible for conducting thorough intake assessments, developing individualized care plans, supervising and training caregivers, and collaborating with team members and families. You will also conduct on-site visits, monitor health status, and participate in discharge planning and educational training for members and caregivers.
About You
Required:
- Current MA Licensed Practical Nurse (LPN) License
- Two years of recent experience with elders or individuals with disabilities
- Strong interpersonal and communication skills
- Ability to work collaboratively in a team environment
- Valid driver's license with an excellent RMV record
Preferred:
- Experience with the Department of Developmental Services or Adult Foster Care Services
- Background in community health or case management
