About the Role
As a Licensed Practical Nurse (LPN), you will be responsible for developing and implementing person-centered treatment care plans, assisting with activities of daily living, administering medications and treatments, supervising C.N.A.s, and ensuring high-quality care in a respectful manner. You will also communicate with healthcare providers and maintain accurate documentation while promoting community integration and advocacy for the individuals served.
About You
Required:
- High school diploma or equivalent required; Associate's degree preferred.
- LPN license in the state of service required.
- Ability to lift at least 25 pounds and perform physically active tasks.
Preferred:
- Experience working with individuals with brain injuries.
- Bilingual/bi-cultural applicants and those with lived experience are preferred.
- One to two years of supervised practical experience in a clinical or rehabilitation setting.
