About the Role
As a Licensed Practical Nurse (LPN), you will be responsible for delivering direct patient care under the guidance of a Registered Nurse, following individualized Plans of Care. Your duties will include documenting patient conditions, managing pain and symptoms, educating patients and families, and collaborating with the interdisciplinary hospice team to ensure high-quality care.
About You
Required:
- Graduate from an Accredited School of Nursing.
- Current NYS LPN Licensure.
- 1-2 years of recent medical surgical experience as an LPN in an institutional setting, home care, or skilled nursing facility.
Preferred:
- Previous hospice/home care experience.
- Basic knowledge of nursing theory and practice, and positive communication skills.
