About the Role

As a Home Health LPN, you will deliver one-on-one skilled nursing care as directed by the RN and physician’s plan of care. You will support the development and execution of individualized care plans, monitor patient progress, and collaborate with the healthcare team to ensure quality care. Additionally, you will educate patients and caregivers on care techniques and participate in the agency's on-call rotation.

About You

Required:

Completion of an accredited practical nursing program.
Current LPN license in the state of Pennsylvania.
Minimum of 1 year of clinical nursing experience.

Preferred:

Home health experience.
A team-oriented mindset and a passion for patient-centered care.
Reliable transportation and valid auto insurance.
