About the Role
As a Licensed Practical Nurse (LPN) in Home Health, you will provide individualized nursing care, teach and supervise patients and their families, and ensure continuity of care. Your responsibilities will include executing physician's orders, documenting patient care, and performing home visits. You will also be expected to comply with department policies and participate in performance improvement programs.
About You
Required:
- Vocational training in an Accredited School of Licensed Practical Nursing.
- Current state LPN licensure.
- Active American Heart Association BLS certification.
- Valid in-state Driver's License with current car insurance.
Preferred:
- Two (2) years of Home Health nursing experience.
- Minimum of (1) year of clinical experience in Medical/Surgical nursing.
- Venipuncture skills.
- V certification.
