About the Role
As a Licensed Practical Nurse in Home Health, you will be responsible for delivering high-quality clinical services, documenting patient visits, and participating in care coordination processes. You will work closely with patients, families, and healthcare teams to ensure effective communication and care delivery.
About You
Required:
- Current and unrestricted LPN licensure in state of practice.
- Current CPR certification.
- Current driver's license and vehicle insurance, access to a dependable vehicle, or public transportation.
- Ability to work flexible hours as required to meet identified client needs.
- Ability to function in any home situation regardless of age, race, creed, color, sex, disability, or financial condition of the client.
Preferred:
- Home care experience.
- Ability to manage multiple tasks simultaneously.
- Able to work independently.
- Good communication, writing, and organizational skills.
