About the Role
As a Licensed Social Worker, you will develop detailed social histories and assessments for residents, create individualized care plans, participate in interdisciplinary care plan conferences, provide support during crises, assist with new resident orientation, coordinate discharge planning, facilitate staff training, and build relationships with external partners to support ongoing care.
About You
Required:
- Bachelor's Degree in Social Work (BSW) or certification as a Social Worker Designee with relevant secondary education.
- Effective communication and interpersonal skills.
