About the Role
As a Licensed Social Worker, you will create care plans that integrate patient and family preferences, assess and treat various psychosocial needs, advocate for resources, and maintain communication with the care transition team to ensure effective health plan implementation.
About You
Required:
- LMSW or LCSW licensure is required.
- High School diploma equivalency with 2 years of cumulative experience OR Associate's degree/Technical degree OR 4 years of applicable cumulative job specific experience.
Preferred:
- Case Manager credentialed from the American Case Management Association (ACMA).
- Case Manager credentialed from the Commission for Case Manager Certification (CCMC).
