About the Role
As a Licensed Vocational Nurse (LVN), you will administer medications, perform wound care, monitor vital signs, and support daily living activities for residents in a non-acute, residential setting. You will work closely with a multidisciplinary team to ensure safe transitions from hospitals to community-based housing, helping clients regain independence and connect to ongoing medical and social supports.
About You
Required:
- Active LVN license in the state of California (in good standing).
- Current CPR/BLS certification.
- 2+ years of experience in direct patient care (home health, skilled nursing, or similar).
- Physically capable of assisting patients with mobility and transfers.
Preferred:
- Experience with Alzheimer's, dementia, or chronic illness management.
- Positive attitude, emotional resilience, and team-first mindset.
