Borinquen Medical Center is seeking a Home Health Social Worker (MSW) to provide compassionate, person-centered home health care in Albuquerque.

As a Home Health Social Worker, you will deliver social work services aligned with physician orders and care plans, assist patients and families with emotional, social, and practical challenges, participate in interdisciplinary care planning and case conferences, coordinate with community resources, and advocate for patient needs. You will also educate patients, families, and team members on psychosocial issues, document visits and care plans, maintain confidentiality, and contribute to quality improvement efforts.
