About the Role

As an LPN in Home Health, you will collaborate with the RN Case Manager and other healthcare team members to ensure effective patient care. Your responsibilities include conducting patient visits, making skilled observations, reporting changes in patient conditions, providing patient education, and completing required documentation accurately and promptly. You will work in a compassionate environment with no weekend or after-hour on-call requirements.

About You

Required:

Graduate of an accredited practical nursing program and licensed to practice in the respective state.
Current CPR or BLS certification with hands-on demonstration.
Minimum of one year of nursing experience.

Preferred:

One year of home care or hospital nursing experience.
Experience with electronic health records (EHR) systems, such as HomeCare HomeBase.
