About You
- Master's degree in Mental Health or Social Work or Graduate from an Accredited School of Nursing.
- 2 – 4 years of related experience in mental/behavioral health.
- 3+ years of case management, care coordination, or discharge planning with adult populations.
- 2+ years of experience in coordinating healthcare/behavioral health services and providing patient advocacy.
