About the Role
As an LTSS Service Coordinator - Clinician, you will assist in identifying members for high-risk complications, provide clinical data to the responsible RN, and participate in care coordination for members with chronic illnesses. You will work closely with the RN and healthcare team to ensure effective utilization of health benefits and develop care plans tailored to member needs.
About You
Required:
- Current, unrestricted LPN/LVN, LSW, LCSW, LMSW or license other than RN in applicable state(s).
- Minimum of 2 years of experience working with individuals with chronic illnesses, co-morbidities, and/or disabilities.
Preferred:
- Case management experience with older adults or individuals with disabilities.
- BA/BS in Health/Nursing.
