About the Role
As a Magnolia Project Case Manager, you will be responsible for coordinating prenatal, pre- and interconception services for high-risk women. Your daily tasks will include assessing risk, developing risk reduction goals, linking participants to services, and ensuring comprehensive Life Course Plans are developed. You will also provide social and mental health support, follow up with providers, and maintain documentation while ensuring confidentiality.
About You
Required:
- Two years experience as a case manager.
- Bachelor's degree in the field of maternal child health or social science preferred, or 5 years professional experience in direct service, social services, maternal and child health or related field.
Preferred:
- Experience working with diverse communities, organizations, and agencies.
- Knowledge of local health and human service providers.
