About the Role
As an MDS Coordinator/LVN, you will be responsible for notifying and coordinating the Interdisciplinary Team (IDT) for MDS assessment completion. Your daily tasks will include coordinating the development, implementation, and evaluation of the plan of care, administering treatments and medications as needed, and ensuring nursing care is provided in accordance with infection control standards. You will also follow safety policies and initiate emergency measures as required.
About You
Required:
- Current LVN License in state of California
- 2 years experience as an MDS Coordinator in a Skilled Nursing Facility
- Background check with no history of abuse
