About the Role
As an MDS Coordinator (LPN), you will conduct and complete the Minimum Data Set (MDS) assessments, evaluate residents' conditions, prepare reports for Medicare and Medicaid, develop care plans, and provide information on financial assistance programs. You will also ensure that MDS notes are descriptive and assist with discharge planning.
About You
Required:
- Must have and maintain a current, valid state LPN license.
- Current, valid CPR certification required.
Preferred:
- Three (3) to five (5) years’ experience working in a MDS or assessment role in a senior residential care, healthcare, senior living industry or long-term care environment.
